Southeastern Spine Center welcomes and looks forward to the opportunity in providing you with the superb clinical expertise and the compassionate care we offer. Our staff will try to accommodate all of your needs and make this experience as easy as possible.

Patients are seen in the office by appointment only. Office hours are Monday through Friday 8:00 am until 5:00pm. All efforts are made to accommodate the initial new patient appointments as soon as possible. You may call the office (941) 371-9773 or email directly with a request to scheduling@SoutheasternSpineCenter.com.

We ask that all new patients bring with them the following at their initial appointment:

  • Picture ID
  • Insurance card or information
  • Films with re ports (x-ray, CT Scans, MRIs)

Both the Pre-Screening Form and the Patient Registration Form are required at your initial appointment. For your convenience, you may download both forms, complete at your leisure, and bring with you. Or, to further expedite your paperwork, you may fill out and submit the Online version of our Pre-Screening Form immediately and bring the Patient Registration Form to you initial appointment.

Worker's Compensation

Those patients that are Workers Compensation cases must have the initial appointment made by the insurance case manager or claims adjustor.

Southeastern Spine Center gladly accepts patients who have work related injuries that deal with a workers compensation claim. This office works closely with the patient, insurance adjuster, and case manager assigned to the case. We welcome case managers to attend appointments with the patient to provide for continuity and expediency of the plan of care. There is not a required pre-payment for case manager visits, as this is directly billed for your convenience.
